Client Rights & Responsibilities

Every person we serve has inherent rights that we are committed to upholding — and a shared responsibility to help us provide the best care possible.

Effective date: September 17, 2026

At A Pearl's Journey Home Care, we believe that knowing your rights is the foundation of a trusting care relationship. This document is provided to every client before services begin. If you have questions, please call us at (463) 202-4773.

Part One

Your Rights as a Client

1

Right to Dignity & Respect

You have the right to be treated with courtesy, dignity, and respect at all times — regardless of age, race, ethnicity, religion, gender, sexual orientation, disability, or any other characteristic. Our caregivers are trained to honor your individuality and personal preferences in every interaction.

2

Right to Privacy & Confidentiality

You have the right to privacy in your home and in all matters related to your care. Your personal, health, and financial information will be kept strictly confidential and will not be shared with third parties without your written consent, except as required by law.

3

Right to Participate in Your Care Plan

You have the right to be fully informed about your care plan and to actively participate in developing, reviewing, and modifying it. Your preferences, goals, and concerns will be respected and incorporated into the services you receive.

4

Right to Refuse Services

You have the right to refuse any service or task at any time without fear of retaliation or negative consequences. If you decline a service, we will document your decision and work with you to find an acceptable alternative whenever possible.

5

Right to Be Informed

You have the right to receive clear, complete, and timely information about the services being provided, any changes to your care plan, caregiver assignments, fees, and agency policies. You will be notified in advance of any significant changes that affect your care.

6

Right to Voice Concerns & Grievances

You have the right to express concerns, complaints, or grievances about your care without fear of discrimination or retaliation. All concerns will be acknowledged promptly and addressed in a fair, respectful, and timely manner. You may contact us directly at (463) 202-4773.

7

Right to Safe & Competent Care

You have the right to receive care from qualified, trained, and compassionate caregivers. Our staff are screened, background-checked, and trained to provide safe, competent, and consistent non-medical personal care services.

8

Right to Freedom from Abuse & Neglect

You have the right to be free from physical, emotional, verbal, sexual, and financial abuse, neglect, and exploitation. Any suspected abuse or neglect will be reported immediately to the appropriate authorities in accordance with applicable law.

9

Right to Continuity of Care

You have the right to receive consistent, reliable care. If a caregiver change is necessary, you will be notified as soon as possible and every effort will be made to ensure a smooth, uninterrupted transition.

And in return

Part Two

Your Responsibilities as a Client

1

Provide Accurate Information

Please provide complete and accurate information about your health status, medical history, medications, allergies, and any changes in your condition. This helps us create and maintain a care plan that truly meets your needs.

2

Maintain a Safe Environment

Please ensure that your home environment is reasonably safe for our caregivers. This includes keeping walkways clear, securing pets, and notifying us of any hazards or conditions that may affect caregiver safety.

3

Treat Caregivers with Respect

Our caregivers come to your home with dedication and care. We ask that all clients and household members treat Agency staff with dignity, courtesy, and respect. Verbal abuse, harassment, or threatening behavior will not be tolerated.

4

Communicate Changes Promptly

Please notify us as soon as possible of any changes in your health, living situation, emergency contacts, or care needs. Timely communication helps us adjust your care plan and ensure your safety.

5

Honor Scheduled Appointments

Please provide at least 24 hours' notice if you need to cancel or reschedule a visit. Consistent scheduling helps us serve all of our clients reliably and ensures your caregiver's time is respected.

6

Stay Within the Scope of Services

Please refrain from asking caregivers to perform tasks that fall outside the agreed care plan or that are beyond the scope of non-medical personal care. If you need additional services, contact us and we will work to update your care plan accordingly.

7

Make Timely Payments

Please ensure that payment for services is made according to the billing schedule outlined in your service agreement. If you have questions or concerns about billing, please contact us promptly so we can address them together.

8

Participate in Your Care

We encourage you to be an active participant in your own care. Share your preferences, ask questions, and let us know when something is or isn't working. Your voice matters and helps us serve you better.
